I’m trying to figure out the best timing for an H. pylori test after taking a PPI. Ragdoll Hit I stopped my PPI about two weeks ago because I was told it could affect the test result. I’m planning to do a stool antigen test, but I’m not sure whether two weeks is enough or if I should wait longer.

For anyone who has been through this, did your doctor ask you to stop PPIs for a specific number of days before the test? Also, if you had a negative result after stopping the medication, did you repeat the test later to make sure it was accurate?

I’d appreciate hearing about your experience, especially if you had previously received treatment for H. pylori and were doing a follow-up test.

While two weeks off PPIs is indeed the standard clinical guideline for both the Urea Breath Test and Stool Antigen Test, the physiological mechanisms involved work slightly differently than described:

How PPIs Cause False Negatives: PPIs directly suppress H. pylori by inhibiting its growth and urease activity through acid suppression and mild direct antimicrobial effects. This temporarily lowers the bacterial load throughout the stomach below the detection threshold of stool antigen or breath tests, regardless of whether other oral bacteria are present.

The Role of Stomach Acid & Oral Bacteria:
Low acid (whether from heavy PPI use or severe atrophic gastritis) can allow non-H. pylori bacteria from the mouth or upper gut to colonize the stomach. Some of these oral bacteria produce their own urease enzyme, which actually risks causing a false positive on a breath test, rather than suppressing H. pylori.

Reflux vs. Gastritis: Severe acid reflux (GERD) does not rule out underlying atrophic gastritis or patchy bacterial colonization in the stomach body. Because PPIs systematically lower the total density of H. pylori, strong acid output prior to stopping the medication does not guarantee that two weeks was enough if bacterial density was significantly suppressed.

Bottom line: If you have been off all PPIs for a full 14 days (and have avoided antibiotics or bismuth compounds for at least 4 weeks), a stool antigen test will provide a reliable result. If symptoms persist despite a negative test, discussing a repeat test or alternative evaluation with a healthcare provider is the safest next step.

From what I understand, PPIs can interfere with H. pylori testing and potentially cause a false-negative result. Two weeks off a PPI is commonly recommended before a stool antigen test, although it’s still best to follow the specific instructions from your doctor or testing lab.

If the test is negative but there’s still a strong reason to suspect H. pylori, it may be worth discussing whether a repeat test or another testing method is appropriate. This seems especially relevant for people doing follow-up testing after previous treatment.

I’d be interested to hear what timing others were given by their doctors as well. And after dealing with the testing details, a quick round of geometry dash can be a nice little distraction.
