Suggestion after failed 2nd h.pylori treatment
I am 21 yrs old male.Its now almost a year i have been suffering from gastritis. About a year ago i was fine. I still remember how i was fine the day before and suddenly the next day during lunch time i felt nauseous and burped a lot. I was unable to take food properly. I thought its a normal gastritis and took pantoprazole for a week but it started to get worse. Then i had endoscopy performed. Endoscopy showed nothing. It was normal. Doc put me on a pantoprazole. I got quite well for 2-3 months. But again the symptoms started. I could not eat well, felt nauseous all the time, lost almost 20 pounds and also burped a lot. I also vomitted many times. It was like a nightmare all the time.Then i went to a doctor(gastroenterologist). I had endoscopy performed along with few other blood and stool tests(not for h.pylori). I was diagonosed with gastritis in the antral and duodoneum region. Doctor prescribed me triple regimen for h.pylori( amoxcilin/clarithyomycin/pantoprazole) for 14 days along with domiperdone tablets as i was feeling nauseous all the time. I took the prescribed medicine but the symptoms never got away. I was feeling worse and worse. After 2 months the sympotms were same. I had endoscopy performed again along with biposy for h.pylori. This time i there was no gastritis and i was treated negative for h.pylori.( Labs in my country are at very poor state). Still the symptoms were same infact worse. I had severe indigestion. Stool is almost watery sometimes. Then i decided to move to the next country for treatment, since the doctor here in my country hardly knew about h.pylori. I had upper gi endoscopy performed along with biopsy and colonoscopy aswell. There i was diagnosed with gastritis in the antral(not sure if its antrum) region . Biopsy for h.pylori turned out to be positive. Colonoscopy was normal. He then prescribe me same triple regime therapy(10 days) for h.pylori. I said i already took the above regime so what's the point in taking the same. He said take take this and we will see it. He also prescribed me probiotic enterogermina capsule and said to take pantoprazole and domiperidone tabs for two months. But after 2 months my symptoms were still the same infact worse.I am now skinny.Also i have back pain due to deficiency of vitamin b12 and d3(probably due to use of ppi for almost a year). I was given some sort of vitamin supplements.So now i am paying visit to the doctor again. So please help me what therapy should i request the doctor, since i think the bacteria is resistant to clarithyomycin and what are the steps should i take? I am getting worse. My study has come to halt for almost a year now. Also please suggest what should i ask doctor like anti microbial sensitivity testing and other tests? I would be grateful for your suggestions and hopefully i will be able to move on from this disaster.

you are right about the resistance. your doctor should not be giving you the same drug. the easy way is to try PPI + amox + metronidazole.
We have recently published a paper on treatments on patients with resistant H. pylori. The eradication rate is quite satisfactory. However, you'll need to discuss with your GP or specialist about the side effects and resistant level in your country.

So what should i request the doc. PARC or PBRC ? What would be more appropriate? Doctors here in my country seem to know very little about the bacteria. I need to take suggestions from you and convince ta doctor about the 2nd line/ 3rd line.. therapy. I would be grateful for your help.

If you are not allergic to Penicillin, I would recommend PARC. It is probably easier to get in your country than Bismuth

Besides this i have terrible nausea. I feel like vomiting all the time and some sort of gas/air is stuck in my pipe. When I burp it eases for minute and again starts to fill up again. Does h.pylori cause nausea or is it other thing thats bothering me? This nausea is really killing me.

yup. it does sounds like H. pylori symptom. as for nausea, you can take some Dopamine antagonists.

i am on domiperidone tablets. But they seem to help very little. Yesterday my doc got me tested for tissue tranglutaminase which came back negative. He thinks h.pylori is gone even without testing by the first line treatment. How do i convince him? Deep trouble

if your H pylori is negative, there is very little you can do about the symptom. Unfortunately the non-helicobacter pylori associated stomach problem is still not very clear. have you consider a second opinion?

suggestion- pls go get the breath test done and see if you are still infected. if not, concentrate on healing your gastritis.
try to find out who provide services for urea breath test and you can approach them directly, if your doctors remain unconvincing. it will help you focus your mind in right direction.

I'll throw my ten-pence worth in here.
First, H pylori symptoms can be caused by many other factors.
Low stomach acid, other bad bugs such as candida and parasites, other bad bacteria, dehydration can all cause similar symptoms, or the same symptoms, as H pylori.
So they all need to be considered.
In working with more than 1200 cases of digestive complaints, I've noted many trends.
Here are some of the really important ones:
- Many people get rid of H pylori without shifting their symptoms.
- Some people get rid of all symptoms without ever needing to get rid of H pylori (e.g. by changing eating habits, etc).
- Some folk need specific protocols to heal gut tissue
- Some need to supplement with betaine HCl to restore digestion
- Some folk need to address nutrient deficiencies such as potassium, magnesium...and their symptoms clear up
- Some people have heavy metal toxicity which accumulates in the neurons of the gut, causing problems
There are many reasons for symptoms over and above H pylori, and HP is often used as a scapegoat.
Some lucky people take triple therapy and feel wonderful.
I help the people who don't!
I also like to change people's mindsets when symptoms don't go away.
So I ask: do you want to get rid of H pylori, or do you want to optimize your health? The two are not interchangeable.
Without addressing food, lifestyle and other possible symptom-causing factors, folk often run up a blind alley.
Let me give you one very big example.
Gluten causes 55 diseases (New England Journal of Medicine).
It's thought that 80% people have a genetic disposition to gluten intolerance on some level.
It sets the intestine on fire, in much the same way as H pylori sets the stomach on fire.
Thus, it can cause similar or the same symptoms as H pylori.
I've had clients with H pylori stop eating gluten and have zero symptoms in 3 days. I am not exaggerating. They didn't need to get rid of H pylori to get rid of their symptoms.
So, taking a more holistic approach when symptoms don't go away is of the utmost importance (in my humble opinion
We're all individuals. We're unique and these factors affect us all in different ways.
I personally had H pylori TWICE, and got rid of it both times using Matula Herbal Formula, which is demonized as a scam on the Internet...but nothing could be further from the truth.
