Hello (thank you for helping).

My son has an immune problem, I won't go into that. He has had symptoms of HP for the past year but doctors didn't recognize it as we were treating other medical problems. Over a month ago we tested for HP thru stool and it was positive. He was put on:

Omeprazole 20 mg - 1 x a day in the morning before BFast
Probiotic - 50 billion
Amoxicillian 500 2 a day
clarithromycin 500 1 a day
Probiotic - 10 billion

His last day of Treatment was Monday of this last week. The first 3 days were really ruff on him, then he started to adjust to the meds. He had some pain and some nausea but not more than usual. And some numbness as he describes it. On the 14th day, last Monday he had extreme nausea. Felt like he was going to throw up all day. He has been out of school all week as he can't get himself off the couch. Says the nausea and lower pain and numbness is too much for him to take. He's not a complainer, so I know he's not doing well.

Doctor suggest that we do another 14 day run of ABXs (Pediatrician). I asked if he could get me into a Gastro as all the referrals that I have gotten are booked for weeks out.

QUESTIONS: Is it normal to run another cycle of ABXs this early? Would he have symptoms after ending a cycle if ABX had rid it of the HP? If so, how long before he should start feeling better? If I were to go ahead with an endoscopy (which I'm very nervous about doing to him) would this for sure tell me what ABX that would kill it? Due to all his abx usage he might have some resistance to them so this would tell me which ones to use? Or is this a guessing game? Any suggestions for meds that would help with the nausea? What about alternative therapies?

I apologize for all of the questions. I'm so lost with this. I wish there was a book I could read that would answer this for me...Hint, hint.

there is a lot of books about H. pylori and I assure you, you wouldnt like to read them. too technical for you. for instance, this is one of my favorites,

I am sorry to hear that your child has to go through the horrible 14 days treatment and still got it positive. Can I know what are the next set of antibiotics the doctor prescribed? How old is your son?

to answer your question about the symptom, it is common that symptoms persist for 1-4 weeks before the individual feel the difference. It is because the stomach take times to recover from the inflammations caused by H. pylori.

Endoscopy is pretty safe. Your child will be sedated and he wont remember much. Remember to request for antibiotic sensitivity test for the biopsy. so you know what antibiotic is right to kill off the bacteria.

alternative therapies? i dont believe anything else that can kill H. pylori other than antibiotics. Chilli, pepper, salt, curry, garlic, etc etc has been shown able to kill H. pylori in the laboratory. but the Asian people who enjoy the spices are the ones with the high infectious rate. So, I dont believe they work in the stomach.

Hello,

He's 10 will be 11 in December. He ended treatment this past Monday, this is when the severe nausea, pain, numbness increased so the doctor prescribed the same regimen above. I READ on the internet YOU SHOULD change the regimen at least by one ABX so that you have a different protocol in the mix. My problem is I read nothing about starting the 2nd treatment 4 days after the last. I show that you stop, retest after a month then proceed to do it again. I worry that it's too soon. As you have stated it's normal for 1 - 4 weeks to feel the same. (thank you for that insight)

We are trying everything we can to get him into a specialist. I have talked this over with my husband and we now believe we need to do the scope to make sure what we are dealing with and also to know up front what ABX are best to kill this thing. It's really killing me to see him suffer with the effects of the medication. He does say that he's better this morning and wants to go to school. Good sign!

So really, it's your opinion that we should give the ABX some time to work. Yes, it's normal to feel the same or a little worse after ending a cycle of ABX. I'm assuming if this was your child you would wait to hear from a specialist? And if we did do the 2nd cycle would YOU suggest changing out one of the meds?

Thank you for your time. Amazing that I can't get in with anyone... from a Mother's perspective my child should be seen right away and everyone should stop and listen... to the rest of the world, life goes on.

Hi worrying mother,

I believe that if 14 days of treatment fail, the bacteria should be resistant to the first treatment. the 2nd treatment should be different.

the standard second treatment is
Amoxy, Metronidazole and PPI

or, a combination of 1st and 2nd treatment
Amoxy + Metronidazole + Clarithromycin + PPI

is your child this weak before the treatment? because i believe the side effects of the medication made him weak.

Was he weak? Hmmm, this past week he was very low in energy. Monday - Thursday he could hardly get himself off the couch. I didn't know what to expect as the doctor PED didn't tell us anything. The first three days were the worst for him, then he adjusted. But the day he ended treatment it was bad all over again.

Today he returned to school and is feeling much better. He had some pain and some nausea but he says it was way less. He looks a lot better. I'm really thankful that you told us that it's normal to have symptoms 1 to 4 weeks after stopping abx. We got a call from the doctor today and they got him an urgent appointment next week. I appreciate your help. :roll:

remember to stop all antibiotic and PPI for this week. those drug will give false negative results during endoscopy.

also, see if you are able to get a CLOtest during the biopsy and antibiotic sensitivity test. CLOtest will tell you immediately if H. pylori still available. antibiotic sensitivity test will take at least 2 weeks, but it will tell you if H. pylori present, what antibiotic it is resistant to.

H.pylori has never been connected with sore throat. The highest level H.pylori can reach is probably C6 which is the upper end of the oesophagus where a 1-2 cm diameter piece of mucosa conatining acid secreting gastric tissue exists in 5% of people - (this is called a "sentinel patch" or "gastric heterotopia") - this can be seen on endosopy sometimes as the 'scope is being removed. However, it is usually not the cause of symptoms and is a normal variant. One could imagine that an H.pylori infection there could lead to swollen lymph nodes and they could cause soreness. Perhaps also inflammation at the esophago-gastric junction could cause swelling and then lead to coughing or burping and finally soreness from the extra muscular activity.
