Second treatment

Hello, this is my first post here and I need help...
I got diagnosed on march with H.pylori by a stool test and I have symptoms like constant burping, stomach pain when its empty and constipation.
My first treatment for 14 days was:
Clarythromicin 500 mg twice a day
Amoxicilin 500 mg four times a day
Esomeprazole 40 mg one time a day
Bismuth tablets twice a day

Unfortunately, the treatment didnt work and I am still positive.
When I went again to the doctor he told me that its possible that the first treatment didnt work because my acid stomach leves were still high, so my new treatment for 14 days is:

Clarythromicin 500 mg twice a day
Amoxicilin 500 mg four times a day
Esomeprazole 40 mg one time a day
Bismuth tablets three times a day

He told me that if this doesnt work I would have to change antibiotics.
I also cant do antibiotics resistance test because they dont do it where I live.

My question is: would be worth to try and do it?. I am a bit scared because the antibiotics side effects + it may not work because I am using the same antibiotics.

Since the following treatment failed
Clarythromicin 500 mg twice a day
Amoxicilin 500 mg four times a day
Esomeprazole 40 mg one time a day

you should not try again the second time.
The bacteria is clearly built resistance to this combination.
Since H. pylori almost never become resistant to Amoxicillin and Bismuth, you check with your doctor about swapping Clarthromycin with Metronidazole
maybe also swap Esomeprazole to Rabeprazole (just in case esomeprazole isnt suitable to you)

It might be worth asking your doctor about switching to a different antibiotic combo, especially since you’re still symptomatic. Hang in there — it sometimes takes a few rounds, but many people do get better!

What are the potential risks and benefits of continuing with this second treatment versus seeking alternative options, crossy roadsuch as changing antibiotics or consulting a specialist?

These are the antibiotics that can be reused, as H. pylori hardly becomes resistant to them..
Amoxicillin
Bismuth compound
Tetracycline
Furazolidone

The following antibiotics can only be used once, as H. pylori almost always becomes resistant to them if exposed.
Clarithromycin
Metronidazole
Ciprofloxacin
Rifabutin
