H Pylori and Loose Stools

Hi Helico Expert,

Iam glad that I found this forum, kudos to the creators and experts helping people deal with H pylori.

I got tested +ve for H pylor in Oct'20. My symptoms include:
  • Pain in the middle part of the chest
    Feeling of stomach fullness
    burping
I started the triple therapy (amoxycillin,metronidazole,nexium) and within 5 to 6 days my condition worsened as I was having diarrhea 10-12 times a day and I stopped the treatment, post which the diarrhea subsided. Then I thought that the pain in the chest region could be due to some other issue and not related to stomache so I visited, cardio, Pulmonologist, Rheumatologist, did various tests and everything came normal. In some time I started having loose stools around 3-4 times a day. Visited the Gastro doctor and he suggested upper Endoscopy and colonoscopy.
Colonoscopy results came back normal -ve for IBD. However upper endoscopy result was "STOMACH, ANTRUM, BIOPSY --- ACTIVE NONATROPHIC CHRONIC GASTRITIS WITH MODERATE ATROPHY, NO INTESTINAL METAPLASIA OR DYSPLASIA. HELICOBACTER PYLORI ARE NOT SEEN"

Post the endoscopy, I started the Omiz+ 40mg for one month and then reduced to Omeprazole 20mg for another month. My main concern of diarrhea was stopped but I started having bright yellow colored urine, ringing ears, sleep problems, muscle twitching, jolts while trying to sleep.
After I stopped the PPIs, I again had soft stools (~4 times a day) intermittently. Its a month and half I stopped PPIs and I still have soft stools on average 3 to 4 times a day and bright yellow colored urine.

I could not understand what could the reason be, I consulted kidney specialist and did various urine exams, and everything came normal.

Can H Pylori will be in the system even when the biopsy result came negative? Can it cause soft stools (yellow coloured) and bright yellow urine?

Help Helico expert!! Your advice is really appreciated..

It seems that the initial triple therapy for H. pylori resulted in severe diarrhea, and subsequent investigations revealed chronic gastritis with moderate atrophy but no H. pylori detected, raising questions about the persistence of symptoms and the possibility of H.

I think "ACTIVE NONATROPHIC CHRONIC GASTRITIS WITH MODERATE ATROPHY" is the hint
usually the term "Active" gives you a hint of some infection. Usually, pathologists are encouraged to look harder for H. pylori if it is "active".

However, "Atrophy" means your acid secretion may be damaged by H. pylori. Also means that your stomach is less acidic and other bacteria can grow and suppress the growth of H. pylori.

Perhaps that's why H. pylori is not seen.

did your doctor ask you to repeat your endoscopy in 6-12 months? I think you should.

continue PPI to ease your symptoms. However, taking PPI will give you false negative breath test. so your options are
1. continue PPI until you feel better. then stop PPI for 2 weeks and repeat breath test.
2. stop PPI for 2 weeks now and repeat breath test
3. switch PPI with H2 blockers for at least 2 weeks, then stop H2 blocker the night before the breath test

I think it's important to find out that if you are still infected with H. pylori.
you may need to take more antibiotics if you still have H. pylori.

Another possibility is that persistent symptoms are not necessarily due to persistent H. pylori but are a consequence of damage to the gastric mucosa, intestinal dysbiosis, or persistent inflammatory overgrowth, something that H. pylori eradication alone sometimes cannot immediately remedy.
