4 years of Chronic Gastritis - Seeking help

Good day and I hope you’re doing well.
Hope you’ve ten minutes to read my story of chronic disease explained here below.
I’m a 30 years old Italian guy, having a good and balanced life in general, doing sports like gym/swimming.
I've lived and worked in Dubai for about 6 years (from 2010 to 2015), now I’m working in Italy since 2016.
While in Dubai I used to travel a lot in different places (mainly countries in Gulf Area/Middle East and North Africa).
On the 29/09/2015 I was coming back from Oman and I started to feel very unwell during my flight back to UAE.
I was feeling stomaches, nausea, dizziness and similar symptoms.
I didn’t eat for about two days (a part of some small snacks and liquids) and after three days my symptoms completely disappeared.
About one month later (24/10/2015) I was going to my office by car and I started to feel again the same symptoms described before.
The day after I woke up (after a normal sleeping with a severe stomaches while feeling again nausea and dizziness.
I’d like to underline that in the same day a kind of severe acne rosacea appeared to my cheeks (both left and right), very strange considering that I had a clean skin for all my previous life.
Hence, I went to the doctor in Dubai which prescribed me some basic blood exams which turned all negatives.
I did also a blood test for Helicobater Pylori with turns negative as well.
After that the doctor prescribed me some PPI (Nexium) and antacids for about two months.
As the treatment didn’t make me feel better I decided to come back in Italy to seek additional medical assistant.
In italy I did my first gastroscopy in which they found a bile reflux into my stomach and chronic gastric.
I was medicated with prokinetic agents (Levopraid/Levobren) + PPI (Omeprazole) and a drug for the bile reflux (called RIOPAN GEL).
From that time my acute gastritis became a chronic gastritis that I’m carrying until today with the following symptoms:
- Indigestion after eating and painful stomach;
- Nausea (especially in the morning);
- Burning stomach (almost 24 h/day);
- Acne rosacea/vulgaris at my cheeks (it never disappeared from the beginning of my gastritis in 2015);
- Diarrea / loose stools (just sometimes), but without having intestine pain or discomfort;
- I do not have acid reflux, so no issue at my esophagus;
Since 2015 I’ve been visited from several specialists which prescribed me different exams.
I summarize here below the main exams:
EXAMS TURNED NEGATIVE
- SEVERAL BLOOD EXAMS (included allergies IgE and IgA for Celiac), all Negative (a part of the ones mentioned later);
- SIBO test, Negative;
- PARASSITE FECAL TEST, Negative;
- TEST for TROPICAL DESEASES (histolytica, Dispar, Cryptosporidium, Dientamoeba, Giardia, Blastocystis sp, Strongyloides, Schistosoma, Hymenolepis), All negative;
- ABDOMINAL ULTRASOUND AND TAC, Negative;
- HELICOBACTER BREATH TEST, Negative;
- COLONOSCOPY, Negative;
- ESOPHAGEAL MANOMETRY + 24HOUR PH MONITORING, Negative;
EXAMS TURNED POSITIVE
- S-Gastrin blood: 11 pg/ml (range 13-115);
- ANCA atypic: POSITIVE++;
- Calprotectin: 88 MG/KG (range 0-50);
- LACTOSE INTOLERANCE, positive and very relevant (despite I’ve eliminated lactose and diary from my diet since more than three years my symptoms remained the same);
In did another gastroscopy last year (October 2018) and the result was:
- REGULAR ESOPHAGUS with hyperemic mucosa (above cardias);
- REGULAR STOMACH with bile and mucus while fasting;
- NORMAL MUCOSA AT THE STOMACH FUNDUS;
- HYPOTROPHIC MUCOSA AT THE BODY AND ANTRUM;
- PRE-PYLORIC HYPERAEMIA;
- DUODENUM WITHOUT LESIONS;
And these biopsies:
1) ESOPHAGUS, regular;
2) BODY OF STOMACH. Chronic inflammation (1+), no glandular atrophy, no Helicobacter;
3) ANTRUM OF STOMACH, Chronic inflammation (1+), no glandular atrophy, no Helicobacter;
4) DUODENUM, regular, rapport villus/crypt equal to 3/1, negative parassite searching;
I did also a GASTROPANEL (while fasting) where only Gastrin-17 was out of the range (they found 0,1 pmol/l against a range of 1-10). I stopped PPI for about one month before doing gastropanel.
As you can understand I’m suffering with a chronic disease that seems to be a never ending story.
I’m kindly seeking your advice/help to understand if you’ve any idea about what is going on my stomach and why my gastrin is too low.
As indicated before, as I’ve travelled in different countries (when sometimes hygienic conditions are not so good) I’m asking you if maybe I got some bacteria (despite all HP tests are negative) which is making my stomach in trouble.
Moreover, I believe that there’s a strict connection to my cheek acne and stomach problems as they appeared all together in the same day. I read that in general skin problems could be related to internal body problems (but many doctors I saw they didn’t take this thing in a serious way).
Of course doctors gave me different prescription (especially different PPIs, antacids, prokinetics) but nothing have worked so far.
Of course some of the doctors addressed my problem to psycosomatic desease due to stress.
I’m actually very low stressed, I don’t feel any other issue other than problems on my stomach, I’m not feeling anxiety/depression.
Despite that I’ve tried a cycle of antidepressant (Paxil) and anti-anxiety meds (Xanax and others) without any benefits.
I never had panic attacks or similar problems, I’m eating and sleeping very well (despite sometimes is difficult due to my stomach issues).
I hope you’ll read this post and really really thank you in advance for your kind support and sorry for my disturb.
I hope to find the right path to solve/find reliefs my stomach problems.
