Dear All, just in case anyone can help me on these questions.

I have been diagnosed with HP since 15 Jul 15. Actually the symptom (pain in right stomach) have already happened within these 2 years. But i just found a doctor can diagnose my disease since this Jul-15. This is the treatment i receive so far:

15 Jul 15
- UBT, Diagnosed with HP
- Treated with 2 antibiotics (cravit and amoxycilin), combined with nexium

21 Aug 15
- UBT, the bacteria is lower but still exist
- Another antibiotics (cravit, tetracyline, amoxycilin, rifampicin), combined with nexium

22 Sep 15
- UBT and endoscopy, the result is negative
- Treated with Dexilant for 1 month

27 Nov 15
- UBT, the bacteria is active again
- Another antibiotics (cravit, tetracyline, amoxycilin, rifampicin), combined with nexium

Now I will have another UBT next week (scheduled on 4 Jan 16). I still feel pain in my left stomach and my right stomach. My last endoscopy shows i have gastritis and duonenitis.

My questions are:
1. Why the bacteria is come back very soon? Is it possible the 3rd testing (UBT, endoscopy) gives a wrong information? The bacteria actually did not die?
2. Is it possible the gastritis in my left stomach and duodenitis in my right stomach heal without any PPI? If yes, is there any estimation, how long it will take?
3. I feel some part of my stomach is hardened when i eat nexium. Is it normal or part of the healing process?

Hi, thanks for your explanation.

Now, I know there are 3 types of UBT:
1. C13 (two steps). This was the one that done by my wife and kids at commercial lab
2. C14 (one step). This is the one that I will take in next year (4 Jan 16) at one of hospital
3. 4 steps. This is the one that I took at Kuala Lumpur

Actually I want to show you the result of 4 steps UBT, but I could not find any way to upload the image in this forum.

Hi,

sorry that i missed the second question. Yes, the gastritis and duodenitis will heal itself IF H. pylori is truly gone.

in regards to the breath test, I know there is a C13 and C14 breath tests where,

in C13, patients are asked to blow the balloon first for initial baseline measure, then only ingest the C13 pill for the second breath collection. If there is an elevation of C13 in the breath, in comparison to the baseline, then it is consider positive. That is a two step breath test.

in C14, patients are asked to ingest the pill and collect the breath for one time. because C14, unlike C13, is not naturally present in human body, you dont need to measure the baseline. So this is a one step breath test.

So, I am not too sure why there is a need for four steps breath test. Maybe there is a new diagnosis that I am not aware of.

Hi, the 4 steps UBT is 4 times breath sample taken through 4 times blowing. The carbon is drinked before the 2nd step.

Could you help on my 2nd question? If the bacteria has gone, could the gastritis and duodenitis heal by itself, without take any PPI like nexium? I have taken this nexium/dexilant from jul-15 to 10 dec 15 (end with the last antibiotic).

I have not heard of a 4 step UBT. Perhaps, you should ask for a C14 UBT, which I think is the most accurate.

Thank you very much for your fast response. I have my wife and kids tested with UBT in Indonesia, but it's 2 step UBT. And the result was negative. Meanwhile, i was tested through 4 steps UBT in Kuala Lumpur. Is the 2 steps UBT not accurate as 4 steps UBT?

I will discuss with my doctor in Indonesia for this antibiotic sensitivy test.

I know what you mean the 4 -step UBT test, and is probably recommended by one of the specialist in the hospital.
Before you take /drink the powdered urease solution, you will take a baseline.
Then drink the urease solution.
Take a timer of the first 10 minutes, then blow into the first sample ballon
Take a timer of the next 10 minutes, then blow into another sample ballon
and lastly take another sample for the last 10 minutes on another ballon

The chart would show a result similar to a any breathe test, but with additional 2 points of the 10th and 20th minutes.

IF H.pylori is eradicated...gastritis and duodenitis would heal. The PPI is needed to reduce the acid secretion payload during your stomach lining healing. All this is one thing, another is taking care of your diet. The recovery formula is not just ONE thing, it has alot of external factors as well.
Everyone has different recovery time. Some recover within a week, some takes 1-2 years.
