				H.pylori 2nd treatment

				I had H.pylori since mid Jan 2013.
Did an endoscopy. The biopsy did not return the result, and the doctor mentioned that the place taken probably did not have.
Might get some spelling wrong on the medicine
I was on 2 weeks Amoxicillin 1g /Esomeprazole (Pariet) 20mg / Itopride (Ganaton ) 0.5mg / levofloxaxin 250mg/ Flupentixol
I got UBT on June 7th, and it return a value 4.5 ( expecting <2.3) ...no idea on those values, stating a failure in treatment.
I'm now back on my 2nd treatment with the following higher dosage and different PPI.
Amoxicillin 1g /Esomeprazole (Nexium) 40mg / Itopride (Ganaton ) 0.5mg / levofloxaxin 500mg / Flupentixol
I'm not sure what is my alternative treatment here if I failed this 2nd treatment again?.
Could you provide a 2nd opinion to me?.
My gut is not feeling entirely worse than Jan , where I had massive bloating on the upper abdomen and discomfort.
I'm on my repeat 2nd treatment day 3, and I'm feeling the slight upper bloat after 2-3 hours of meal. I'm not sure if this is normal, as I understand this might be due to the 2nd treatment.

				i wouldnt recommend levofloxacin for your 2nd trial. i dont see how increasing the PPI dosage will help. perhaps you can ask your doctor to switch levofloxacin to tetracycline? or rifabutin?
if you are already taking the 2nd treatment, then finish it and see how it goes. if fail again, perhaps ask if your doctor able to swap levo to tetra or rifa.

				Hi helico_export, appreciate and grateful for your response.
I have actually mentioned to my gastro specialist that he has prescribed to me similar antibiotics, but he wishes to increase my dosage and provide a different and higher dose of PPI. Hence, I proceed on with my 2nd re-treatment.
I will keep in mind on the 3rd (hopefully no need to) option you have mentioned.
I am probably one of those small percentage that did not response to levo 250mg. I seem to see most patients are directly prescribed 500mg,even on medspace articles.
I will report back perhaps 2-3 months later to share my hopefully final battle with this horrid bacteria.

				1 month has elapse.
Currently reducing the dosage of PPI, and hopefully able get rid of the H.pylori on the next Breathe test. Hopefully next month.
I am have been having soft stools for the past 5 days, with limited change in my diet. Am not sure if it's a bad sign or some sudden side effects from medication.
Some slight abdomen bloat, but I am already off the anti-biotics for 3-4 weeks already.
I'm taking myself off from lactobacillus intake at the mean time, and off any dairy products.
Is this normal, as in sudden soft stools (non watery, shapeless) ? on otherwise, I do not feel as worse as I use to be. Any possibilities?.

				An update to the high dosage of PPI on my 2nd treatment resulted in some unknown changes in my bowel movement.
I have been having at least short constipations each week. The stools have been little and at times diarrhae-like, eventhough I kept my normal diet. I do not have a high fibre intake, but I do have my regular vege intakes on meals.
The change in my bowel happened in the beginning of July, with just a sudden change from soft, formed stools to diarrhae like.
My returned back to my gastro this week and was suggested to now do a colonoscopy test.
I have read some articles regarding the use of PPI and how it also creates a different thought about causing bacteria overgrowth and other possible side effects. I'm wondering if that is what I might have been inflicted now during my high dosage intake, with a newer symptom....IBS-?Chrons Disease?..
I'm current taking Forlax, but I do not think it is helping, furthermore, I hate using laxatives to promote my current unknown constipations.
I'm hoping that this are all caused indirectly from the H.pylori  infection.... and that I won't be getting another symptom to deal with.
This is seriously making my quality of life miserable...I just want to get back my normal bowel movement!...sigh.

				i assume you have already completed your second treatment. you are not taking anymore antibiotics now. did your doctor say when you can stop your PPI? there is no literature, but i believe when someone is on PPI, the stomach acid is reduced and other bacteria can grow. it may sometimes lead to other problems. Perhaps stopping PPI will bring you back to normal. and once you stop your PPI, you can then redo your breath test and see if you are still infected.

				Yes, I have completed my 2nd treatment about 2 months now, and starting to reduce my PPI intake to twice per week.
My doctor has not official taken me off the PPI, but I have already stop taking it for a week since my 3rd episode of constipation.
And by what I mean "constipation" is my usual daily bowel has not gone to 2 days, and with diarrhae-like and low volume, which concerns me.
The doctor has instead prescript to me Forlax 4000 for twice daily to hopefully get my bowel moving, but it seems not much difference after 3-4 days.
I had to get a Fleet enema treatment yesterday to get whatever moving. Could be IBS, could be stress, could be anything.
If things does not change I might opt for the colonoscopy to eliminate the possibilities.
Yes, I'm probably schedule for the UBT around next month end...but I'm hoping to get my normal bowel back, which at this point is more important to me. sigh.

				sometimes antibiotics do reset the bacteria composition in your intestine. hopefully your problem will subside through time. looking forward to seeing your UBT result.

				Over the course of month, alot of things has happened.
I start developing constipation, which route me to a colonoscopy test, and came back with a norm result.
I start taking fruits to make my bowel return, and so far...it is coming back slowly.
So, my breathe test came back positive again, with the higher dosage 2nd treatment, which I believe most of you would not recommend. Sadly...it was true. 
I was still having H.pylori.
The result also returned with a high presence of bacterium from the scale, of 5.4 instead of my previous 4.5.
With great sadness....
So, the next dosage is as follows, seems like I'm taking quadruple now.
For 7 days treatment. ( not sure if that is enough, read that it should be 10 days, but my 4x times a day dosage seems heavy ).
Bismuth  4x times a day
metronidazole  4x times a day
PPI  20mg  2x times a day
tetracycline  4x times a day
Wondering if this is a good efficacy compared to my previous levoflaxacin antibiotics.
Update: Ok, I see that I have bee prescribe "Pylera" regimen....sigh.
Also, wondering, if I should take some probiotics supplements, should be taking it 2 hours after the medication?.
Read alot of side effects on this treatment, such as black stools, urine, metalic taste in the mouth...etc...and being warned that metronidazole induces abdominal cramps.
