Treatment

The most popular treatments consist of two antibiotics (one of which is usually clarithromycin) plus a strong acid blocker such as omeprazole (PriLosec, Nexium, Somac, Zoton, etc.). Treatment is for 7-10 days. Cure rate is about 85%

Hello,
I tested positive for Helicobacter Pylori a few months ago.
The first therapy was Pantropazol, Amoxicillin, and Metronidazole for 5 days, then I waited for a month and did a urea breathing test and it tested positive.
Now my doctor prescribed me Pantropazol and Amoxicillin for 5 days, then I should continue taking Pantropazol with Metronidazole and Clarithromycin for the next 5 days and after that just Pantropazol for another 20 days.
So now reading a lot about it, my curiosity is awakened, is this the right treatment?

20 days is a bit too long. I think if it works, it works. there is no need for 20 days. maybe 10 days is enough.

it's important to spread out your medication.

you can also add bismuth to boost your treatment.

now I started taking Metronidazole and Clarithromycin and i feel nauseous, is that good or bad? And there is an aftertaste in my mouth from the pills for hours, even after brushing my teeth.
During this whole time, since being infected with HP i don't have any pain, my only simptome is bloating but just at evening and during the night, I can't even lie on my stomach.

the "bitter", "metallic" taste is because of clarithromycin. The antibiotic get absorbed into your blood stream and flow through your whole system, including saliva. It reminds you that you have antibiotics in your saliva and it's killing all the surviving HP in the mouth (if there is any).

just make sure your PPI dose is high enough. Lots of doctors only give minimum dose of PPI and it's ruining the treatment.

what would be Your recommendation for the next treatment, because the first two treatments failed.
And what do you consider under high dosage of PPI. My doctor prescribed me always 40mg Pantoprazol once in the morning.

i think a good dose of PPI would be Rabeprazole 20mg three times a day.
multiple dose of PPI works better than a single dose.

since you had taken amoxicillin before, you are not allergic to it. So you can repeat amoxicillin.
since HP cannot become resistant to bismuth, you should also include that in your next treatment.

finally, an antibiotic that you had not tried. For example ciprofloxacin or rifabutin.

therefore
Rabeprazole + Bismuth + Amoxicillin + Ciprofloxacin

I bore even myself with all of these questions, but could you write to me the whole treatment.
I know I should take Rabeprazole 20mg three times a day, but how about other drugs?

We normally prescribe
rabeprazole 20mg TID
bismuth 240mg QID
Amoxicillin 1g TID
Ciprofloxacin 500 mg BID

Please make sure you check this with your family doctor. they are responsible for your overall well being, including the side effects from the antibiotics.
