Re: Treatment of H. pylori after two treatment failures

Hi Joanna, Please read the article linked at the end of my post. Your options are limited however I suggest: maximise the dose of PPI, reuse amoxicillin (those old rules about interference between antibiotics are not especially relevant to Hp), consider bismuth/tetracycline/furazolidone/amoxicillin/...

Re: Which is the most suitable treatment for my gastritis?

This type of mild gastritis can be the remnants of past Helicobacter. It gradually improves over several years but never completely resolves. Think of it like sun-damaged skin. There is no specific treatment so, H2 blockers, proton pump blockers and antacids are all OK. Actually, the symptoms are mo...

Re: How accurate is the HP breath test. Thanks

If your symptoms persist use an acid blocker for a few weeks. If no better then you should have an endoscopy because you probably don't have an Hp problem causing your symptoms. Sometimes acid returns to normal or high levels after Hp eradication and you can have more symptoms for a while although y...

Re: Accuracy of Breath Tests (UBT)

You are correct in that the test tube system for the C13 UBT is not foolproof. However, if the test tubes do not contain enough CO2, as sometimes happens, the machine will reject the sample and not give a result. The actual amount of breath collected is irrelevant because the machine measures a chan...

Re: Gastric Bypass patient HP positive?

If you have had the stomach rearranged then breath tests probably won't work for Hp. You would need to rely on Stool antigen and serology, or careful biopsy study during endoscopy. If these tests come back negative then it is unlikely that you have Hp. Most likely you will need to be managed by a ga...

Re: To Dr Marshall please

re False negatives tests: Regardless of the strains, the serology (IgG) is usually quite sensitive - about 95%. For each country and age group there may be a preferred test. So a serology developed in USA might not be quite so accurate when used in Japan. There are many strains of H.pylori, and each...

Re: questions about h pylori medication

Re Cymbalta and trazodone; Cliff, I am not an expert on these two drugs. I believe that clarithromycin can affect other drug metabolism. I suggest you study the relevant manufacturers' web sites - that is what I would have to do but I don't have time today. I can recall a similar question a year or ...

Re: Breath test after treatment-? for Dr. Marshall

Borderline Breath Test We occasionally do see a borderline result. In most cases, a test which starts > 1000 and ends up <200 will be a negative result. Dr Dye is being thorough. I agree, just repeat the breath test, but make sure you are at least 1 month away from antibiotics and follow all the rul...

Re: HP & ITP

Just focus on eradication of Hp then see what happens after that. I recall that there is a new publication about this where the actual antibody / tissue antigen cross reaction was identified. It would help predict your outcome better I expect. You will have to look for that yourself as I am busy thi...
